Shock severity and hospital mortality in out of hospital cardiac arrest patients treated with targeted temperature management

Tabi M, Burstein BJ, Ahmed A, et al. - Given that shock in patients resuscitated following out of hospital cardiac arrest (OHCA) is related to an elevated risk of death, researchers undertook this retrospective cohort analysis to ascertain the links between lactate level, mean arterial pressure (MAP), and vasopressor/inotrope doses with mortality. Participants were adult patients who received targeted temperature management (TTM) for OHCA of presumed cardiac etiology, between December 2005 and September 2016. Findings revealed that an increased rate of hospital mortality was reported in relation to greater shock severity, as reflected by higher lactate concentrations, mMAP < 70 mmHg, and higher vasopressor needs during the first 24 h, in patients who received TTM post-OHCA.
