Sexual function after pelvic organ prolapse surgery: A systematic review comparing different approaches to pelvic floor repair

Antosh DD, Dieter AA, Balk EM, et al. - Researchers conducted this systematic review with the aim to compare sexual activity and function before and after prolapse surgery between specific strategies to pelvic organ prolapse (POP) surgery including native tissue (NT) repairs, transvaginal synthetic mesh (TVM), biologic grafts (BG), and sacrocolpopexy (SCP). From MEDLINE, Embase, and clinicaltrials.gov databases, they screened 3,651 abstracts and identified 77 original studies. Moderate to high quality of evidence was gained overall. A total of 26 studies compared TVM vs NT, 5 compared SCP vs NT, 5 compared TVM vs SCP, and 7 compared BG vs NT. Most robust comparisons regarding sexual function were thus made between TVM and NT repairs and showed similar prevalence of sexual activity, de novo dyspareunia, and sexual function scores. Relative to SCP, higher total dyspareunia is reported after TVM. Although there are sparse sexual function data in the other comparisons, no other differences were identified in sexual activity, dyspareunia, and sexual function score change.
