Septic revision total hip arthroplasty is not adequately compensated by work relative value units

Quan T, Best MJ, Gu A, et al. - This study was attempted to correlate the work effort between aseptic and septic revision hip cohorts, and ascertain whether physicians are appropriately compensated. Researchers obtained data through the National Surgical Quality Improvement Program database for the years 2005 to 2018 to distinguish all aseptic revision total hip arthroplasty (rTHA) cases and septic rTHA cases. They evaluated work relative value unit (RVU), operation time, RVU per minute, and dollars per minute between the aseptic and septic revision hip cohorts. For the study, univariate and multivariate analyses were applied. Physicians are not appropriately reimbursed for this challenging procedure, although rTHA for infection is more complex and requires longer mean operative time than aseptic rTHA. For septic rTHA, this inadequate RVU-based reimbursement may deter physicians from conducting these procedures, which could lead to reduced access to care for patients in need of rTHA for infection.
