Salivary cortisol in the diagnosis of adrenal insufficiency: Cost efficient and patient friendly

Langelaan M, et al. - Researchers sought to explore the role of estimating the salivary cortisol level when adrenal insufficiency (AI) was suspected with the intention of reducing the numbers of ACTH stimulation tests. As per the outcomes, patients without AI displayed maximal salivary cortisol concentrations of 12.6-123.4 nmol/L (95th percentile) after stimulation, compared to 0.5-15.2 nmol/L in AI patients. The numbers of ACTH stimulation tests could be reduced in patients suspected of adrenal insufficiency via a novel diagnostic algorithm, including early morning salivary cortisol analysis. It was deduced that this patient-friendly method could reduce total health care costs.
