Safety and prognostic value of vasodilator stress cardiovascular magnetic resonance in patients with heart failure and reduced ejection fraction

Circulation: Cardiovascular Imaging Sep 05, 2020

Pezel T, Sanguineti F, Kinnel M, et al. - Researchers focused on the prognostic worth of vasodilator stress perfusion cardiovascular magnetic resonance (CMR) in patients with HFrEF (heart failure with reduced left ventricular ejection fraction <40%). This study involved consecutive patients with HFrEF prospectively referred for vasodilator stress perfusion CMR. The participants were observed for the development of major adverse cardiovascular events (MACE), defined by cardiovascular mortality or nonfatal myocardial infarction. The good tolerability of stress CMR was evident without any adverse events. A significant correlation of inducible ischemia and late gadolinium enhancement with MACE development was revealed in Kaplan-Meier analysis. A higher incidence of MACE was independently predicted by inducible ischemia, in multivariable Cox regression. Based on the findings, experts concluded the safety as well as the good discriminative prognostic value of stress CMR in the prediction of MACE occurrence in patients with HFrEF.
