Routine portal vein resection for pancreatic adenocarcinoma shows no benefit in overall survival

Klein F, et al. - Researchers analyzed the impact of portal vein resections (PVR) without evidence of tumor infiltration in the final histopathological examination (“false negative” resections) during extended pancreatic resections on the operative outcome and long-term survival. Survival analysis revealed significantly shorter median survival in the PVR group compared to the SPR group. In addition, patients in PVR group significantly more frequently encountered postoperative complications ≥ Clavien III. Findings thereby favor radical resection affording the best chance for long-term survival in patients with pancreatic adenocarcinoma (PA). Hence they do not recommend a routine resection of the portal vein as recently proposed.
