Role of dietary K+ in natriuresis, blood pressure reduction, cardiovascular protection, and renoprotection

Gritter M, et al. – In this brief review, researchers intended to gain insight into the current knowledge base of the influences of dietary K+ on blood pressure, cardiovascular health, and renal health. They also attempted to offer a mechanistic framework for these effects. A beneficial effect of K+ has been suggested by an overwhelming evidence; however, some studies have reported adverse outcomes. The possibility that a diet high in K+ is renoprotective represents an opportunity for patients with chronic kidney disease (CKD). Patiromer and sodium zirconium cyclosilicate are being recognized as novel oral treatments that permit continuation of renin-angiotensin inhibitors—and the related cardiorenal benefits—while preventing hyperkalemia. The potentially beneficial effects of dietary K+ in patients with CKD could be limited by attenuating intestinal K+ absorption through these binders. After all, a high K+ diet has been recognized as a powerful nonpharmacological approach to lessen the burden of cardiovascular disease.
