Risk stratification for hydronephrosis in the evaluation of acute kidney injury: A cross-sectional analysis
BMJ Open Aug 18, 2021
Tummalapalli L, Zech JR, Cho HJ, et al. - A risk stratification framework for hydronephrosis was externally validated. For the assessment of acute kidney injury, clinical decision support systems may be beneficial to supplement clinical judgment.
Researchers recruited a total of 281 patients in the study cohort. In this study, 111 (40%), 76 (27%), and 94 (33%) patients were in the high-risk, medium-risk, and low-risk groups for hydronephrosis, respectively, based on the risk stratification framework.
They found hydronephrosis in 35 patients (12%). For distinguishing hydronephrosis, the high-risk group had 86% sensitivity and 67% specificity.
In 168 (60%) patients, a nephrology consult was involved and renal ultrasound (RUS) was recommended by the nephrology service in 95 (57%) cases.
Nine (56%) of the 16 total patients with hydronephrosis were recommended to obtain a RUS among patients with a nephrology consultation.
