Risk of mature B-cell neoplasms and precursor conditions after joint replacement: A report from the Haematological Malignancy Research Network

Kane E, Painter D, Smith A, et al. - Given that links between prior joint replacement and B-cell lymphoid malignancies have been reported, but little attention has been paid to links with the disease subtypes, and therefore, to address this issue, researchers analyzed joint replacements from linked hospital inpatient records, utilizing a UK-based register of haematological malignancies and a matched general population-based cohort. This study included cases diagnosed 2009-2015 who were aged 50 years or more. Overall 8,013 mature B-cell neoplasms were included. They used conditional logistic regression to calculate the odds ratios and 95% confidence intervals relative to 10 age- and gender-matched controls. Findings revealed an association between having had a joint replacement prior to diagnosis and myeloma and monoclonal gammopathy of uncertain significance (MGUS). The MGUS risk remained, raised where two or more joints were replaced, with hip or knee replacements, eliminating replacements in the year prior to diagnosis. They also found links with classical Hodgkin lymphoma and two or more replacements or hip replacements; and between diffuse large B-cell lymphoma and knee replacements. For the first time ever, a link between joint replacements and MGUS was evident. Low absolute risks of disease not of major public health concern were observed.
