Risk factors for treatment failure of fracture related infections

Horton SA, Hoyt BW, Zaidi SMR, et al. - Given the potentially devastating effects of infection after fracture fixation and often ineffectiveness of surgical management in their clearance, researchers herein sought for the factors, if any, linked with treatment failure after operative management of a deep surgical site infection. Performing a retrospective review of the surgery scheduling system at a Level I trauma center, they identified 451 patients treated for deep surgical site infection after fracture fixation between March 2006 and December 2015. Initial surgical management failed in 35% of patients in this cohort, necessitating return to the operating room. Polymicrobial infections, open IIIB/IIIC fractures, and removal of implant were identified to be predictive factors for failure. Overweight and obesity were noted to be associated risk factors for failure.
