Risk factors for severe outcomes in people with diabetes hospitalized for COVID-19: A cross-sectional database study

BMJ Open Jul 26, 2021

Ortega E, Corcoy R, Gratacòs M, et al. - A cross-sectional study was conducted to explore the risk of severe in-hospital complications of patients admitted for COVID-19 and diabetes mellitus (DM). Researchers used pseudonymized medical record data provided by six general hospitals from the HM Hospitales group in Spain. They used multiple logistic regression analyses to distinguish variables associated with mortality and the composite of mortality or invasive mechanical ventilation in the overall population and stratified for the presence or absence of DM. They conducted spline analysis on the entire population to evaluate the association between glucose levels at admission and outcomes. They distinguished 1,621 individuals without DM and 448 with DM in the database. The outcomes validate the high burden associated with DM in patients hospitalized with COVID-19 infection, especially among men, the elderly, and those with impaired kidney function. Furthermore, hyperglycemia on admission was strongly correlated with poor outcomes, implying that personalized optimization could help to improve outcomes during the hospital stay.
