Risk factors for secondary poor graft function after bone marrow transplantation in children with acquired aplastic anemia

Pediatric Transplantation Sep 07, 2020

Hama A, Muramatsu H, Narita A, et al. - Given that PGF represents a major reason for cytopenia following hematopoietic stem cell transplantation, and that post-introduction of the FLU/CY regimen, a raised incidence of PGF, especially sPGF, has been observed in children with acquired AA, so, researchers analyzed clinical data of 49 patients (median age, 11 years; range, 1‐19 years) with AA who underwent allogeneic BMT at Nagoya University Hospital from 1997 to 2016, to ascertain the risk factors for sPGF. The development of sPGF was evident in 7 of 49 patients included, and the 5‐year CI was estimated to be 0.15 (95% CI, 0.04‐0.25). The FLU/CY regimen was administered to five patients, and a significantly higher 5‐year CI of sPGF was noted in patients who received the regimen vs in those who were conditioned with the non‐FLU/CY regimen. Overall, in patients with AA, the FLU/CY regimen was shown to confer a risk for the development of sPGF. For children with AA, the establishment of the optimal conditioning regimens is warranted.
