Risk factors for mortality after endovascular repair for traumatic thoracic aortic injury

Journal of Vascular Surgery Jul 25, 2018

Mohapatra A, et al. - Researchers investigated the risk factors and sought to create a risk predictive model for mortality after thoracic endovascular aortic repair (TEVAR) based on available preoperative clinical data. Patients with trauma as the indication for repair were identified querying the Vascular Quality Initiative (VQI) TEVAR data set from April 2011 to November 2017. As endovascular repair is available, they identified a trend toward treatment of milder grade thoracic injuries in the VQI. Independent predictors of mortality identified were age ≥60 years, creatinine concentration ≥1.2 mg/dL, Glasgow Coma Scale score <15, male sex, left subclavian artery (LSCA) involvement (but not whether it was covered), and concomitant facial trauma (but not head and neck trauma). A model based on these variables strongly predicts in-hospital mortality and may help selecting appropriate patients and risk stratification.
