Risk factors for children requiring adenotonsillectomy and their impact on postoperative complications: A retrospective analysis of 2,000 patients

Gehrke T, et al. - Researchers examined how obstructive sleep apnoea and other risk factors influence peri-operative complications in children requiring adenotonsillectomy via performing a retrospective case-control study including 1,995 patients treated between January 2009 and June 2017. Risk factors for complications during or after surgery comprised young age, low body weight, obstructive sleep apnoea, preexisting craniofacial or syndromal disorders and adenotonsillectomy, compared with adenoidectomy alone. Obstructive sleep apnoea plus at least one more of these risk factors were evident among all 13 patients suffering from complications more than 3 h postoperatively. Hence several criteria may assist in preoperative identification of patients at risk of postoperative complications and postoperative monitoring using pulse oximetry overnight is recommended for these high-risk patients. They suggest performing postoperative observation on a surgical ward for all other patients without extra monitoring. They recommend reserving admission to pediatric intensive care for patients suffering serious intra-operative complications.
