Risk factors associated with myasthenia gravis in thymoma patients: The potential role of thymic germinal centers

Lefeuvre CMJ, Payet CA, Fayet OM, et al. - In this detailed retrospective analysis, researchers looked for differences between patients with thymoma-associated Myasthenia Gravis (MG) (MGT, n = 409) and thymoma without MG (n = 111) vs nonthymomatous MG patients (MG, n = 1,246), to determine the parameters, both histological and biological, to enable early diagnosis of thymoma patients susceptible to developing MG. In thymoma patients, the identification of anti-acetylcholine receptor (AChR) antibodies, development of B1 or B2 thymoma subtypes, presence of ectopic thymic germinal centers (GCs), local invasiveness of thymoma, and being a woman under 50 years old were all discovered as parameters related to MG development. GCs were exhibited by 58.8% of MGT patients, with a positive correlation seen between the number of GCs and anti-AChR titers. Immunohistochemistry revealed the presence of thymic GCs in the adjacent tissues of thymomas encircled by high endothelial venules that could favor peripheral cell recruitment. Overall, the histological, especially the presence of GCs, and biological parameters that would facilitate the assessment of the likelihood of the MG outcome after surgery in thymoma patients can be clearly recognized via these analyses.
