Risk factors and outcomes of revisions on arteriovenous hemodialysis access

Rockley M, et al. - Researchers conducted a retrospective cohort study of all patients undergoing chronic hemodialysis (HD) for more than 3 months in the Champlain NephroCare network to determine the risk factors and durability of revision on arteriovenous access (AV) access sites for patients on chronic HD. Description of 2,018 eligible AV access creations in 787 patients was identified. These were composed of 1,808 arteriovenous access using a fistula (90%) and 210 arteriovenous access using a graft (10%). Among AV, at least one revision was required in 67% and there were 12% that received two or more revisions. These revisions sustained limited durability after the procedure. Revisions were most commonly angioplasty (95.8%). Repeated revisions, especially on lower arm, AVG, or occluded access sites, seem to be particularly futile.
