Right heart catheterization in cardiogenic shock is associated with improved outcomes: Insights from the Nationwide Readmissions Database

Ranka S, Mastoris I, Kapur NK, et al. - Right heart catherization (RHC) conferred improved outcomes and increased use of downstream advanced heart failure therapies when used to treat cardiogenic shock.
A total of 236,156 patient hospitalizations with cardiogenic shock were identified from the Nationwide Readmissions Database, and 25,840 patients (9.6%) underwent RHC on index admission.
Lower death (25.8% vs 39.5%) and stroke rates (3.1% vs 3.4%) during the index admission, as well as lower 30-day readmission rates (18.7% vs 19.7%) and death on readmission (7.9% vs 9.3%) were found in the RHC group.
RHC was related to lower index admission mortality (odds ratio, 0.69), lower stroke rate (odds ratio, 0.81), lower 30‐day readmission (odds ratio, 0.83), and higher left ventricular assist device implantations/orthotopic heart transplants (odds ratio, 6.05) during rehospitalization, following adjustment.
Outcomes were not meaningfully different following removal of cardiac arrest patients.
