Right heart catheterization in cardiogenic shock is associated with improved outcomes: Insights from the Nationwide Readmissions Database

Ranka S, Mastoris I, Kapur NK, et al. - Right heart catherization (RHC) conferred improved outcomes and increased use of downstream advanced heart failure therapies when used to treat cardiogenic shock.

  • A total of 236,156 patient hospitalizations with cardiogenic shock were identified from the Nationwide Readmissions Database, and 25,840 patients (9.6%) underwent RHC on index admission.

  • Lower death (25.8% vs 39.5%) and stroke rates (3.1% vs 3.4%) during the index admission, as well as lower 30-day readmission rates (18.7% vs 19.7%) and death on readmission (7.9% vs 9.3%) were found in the RHC group.

  • RHC was related to lower index admission mortality (odds ratio, 0.69), lower stroke rate (odds ratio, 0.81), lower 30‐day readmission (odds ratio, 0.83), and higher left ventricular assist device implantations/orthotopic heart transplants (odds ratio, 6.05) during rehospitalization, following adjustment.

  • Outcomes were not meaningfully different following removal of cardiac arrest patients.
