Resolved vs confirmed ARDS after 24 h: Insights from the LUNG SAFE study

Madotto F, et al. - In this secondary LUNG SAFE analysis, researchers compared outcome in patients with resolved vs confirmed ARDS after 24 h, identified the subgroups with substantial mortality risk and determined the utility of day 2 ARDS reclassification. A high mortality rate of ARDS was reported for both groups. In both groups, age, non-respiratory SOFA score, lower PEEP and P/F ratio, higher peak pressure and respiratory rate were each found to be related to mortality. For mortality prediction, ARDS reclassification at day 2 had limited value. Severe confirmed ARDS carried a substantial mortality risk, suggesting that complex interventions might best be tested in this population.
