Resistant hypertension in pregnancy: How to manage?

Bortolotto MR, et al. - Researchers discussed how to best manage pregnant patients with preexisting resistant hypertension and those who develop severe hypertension in gestation and puerperium. Severe hypertension in pregnancy is mainly due to preeclampsia. They suggested performing differential diagnosis with secondary or primary hypertension. Pharmacological therapy adjustment might be required for women with preexisting resistant hypertension. Apart from angiotensin-converting enzyme inhibitors and angiotensin II receptor antagonists, several drugs including methyldopa, beta-blockers, and calcium channel antagonists can be used to treat severe hypertension. They recommend considering the possible etiology, the fetal well-being, and the mother’s risk while managing resistant hypertension in pregnancy.
