Renal pathologic spectrum and clinical outcome of monoclonal gammopathy of renal significance: A large retrospective case series study from a single institute in China
Nephrology Jul 18, 2019
Yu XJ, et al. - Researchers studied patients with renal biopsy-proven monoclonal gammopathy of renal significance (MGRS), to better understand the pathological spectrum and prognosis of this disease condition. They enrolled overall 187 patients; amyloidosis cases constituted 77% of MGRS cases. Monoclonal immunoglobulin deposition disease cases were 18 (9.6%). Proliferative glomerulonephritis with monoclonal immunoglobulin (G) deposits was present in 10 patients (5.3%), cryoglobulinemic glomerulonephritis in 7 (3.7%), light chain proximal tubulopathy in 5 (2.6%), fibrillary disease in 2 patients (1.1%), and C3 glomerulonephritis was detected in 1 patient (0.5%). The identified risk factors for end stage renal disease included low estimated glomerular filtration rate, reduced blood pressure, and presence of cardiac involvement. A wide spectrum of pathologic lesions was identified in MGRS-an uncommon form of hematologic disorder associated renal injury. The most common type was amyloidosis. Improved survival was experienced by amyloid patients in relation to treatment with chemotherapy and/or high dose melphalan/autologous peripheral blood stem cell transplantation.
