Relationship between the early initiation of insulin treatment and diabetic complications in patients newly diagnosed with type 2 diabetes mellitus in Korea: A nationwide cohort study

A cohort study of newly diagnosed type 2 diabetes mellitus (T2DM) patients, in which the Korean National Health Insurance Service database was utilized, to assess the link between early insulin initiation within a year post-T2DM diagnosis and the risk of diabetic complications.
In this cohort, 52,188 and 1,804 patients were treated with OAD and insulin, respectively, and post-application of propensity score matching (1:1), there were 534 patients in each group.
Relative to the OAD group, a significantly higher risk of overall microvascular complications was observed in the insulin group (hazard ratio, HR=1.48).
There were no elevated risks of overall macrovascular complications (HR=0.90) and all-cause death (HR=1.06).
