Relationship between norepinephrine dose, tachycardia and outcome in septic shock: A multicentre evaluation

Domizi R, Calcinaro S, Harris S, et al. - Among septic shock patients needing norepinephrine for more than 6 h despite adequate volume resuscitation, whether tachycardia is related to mortality, was determined in this multicentre retrospective observational analysis. Participants included 730 adult patients in septic shock, recruited from eight European ICUs. Researchers selected three time points: T1 (first hour of infusion of norepinephrine), Tpeak (time of highest dose during the first 24 h of treatment), and T24 (24-h post-T1). The overall ICU mortality was estimated to be 38.4%. Patients needing high-dose (≥ 0.3 mcg/kg/min) vs low-dose (< 0.3 mcg/kg/min) norepinephrine at T1 and T24 had higher mortality. Overall, poor outcomes were reported in septic shock patients in correlation with the use of high-dose norepinephrine and concurrent tachycardia.
