Reintroduction failure after negative food challenges in adults is common and mainly due to atypical symptoms

Versluis A, Knulst AC, van Erp FC, et al. - Given the reintroduction of food following negative food challenge (FC) encounters many obstacles, so, researchers determined the frequency, causes as well as risk factors of reintroduction failure in adults in this prospective analysis. This study included 80 patients, with 113 negative FCs. Standardized follow-up care was provided to adult patients following negative FCs including a reintroduction scheme and supportive telephone consultations. Telephone interview (2 weeks after FC) and questionnaires (at baseline and 6 months after FC(s)) were performed to obtain data. Findings revealed that reintroduction failure was commonly encountered after negative FCs in adults, and there was a rise in reintroduction failure over time and the main reason for such failure was atypical symptoms. This calls for more patient-tailored care prior to and following negative food challenges. For long-term reintroduction failure, 5 risk factors were identified: if culprit food was not one of the 13 EU regulated allergens, reintroduction failure at short-term, atypical symptoms during FC, lower quality of life and higher state anxiety.
