Rehabilitation after total knee arthroplasty: Do racial disparities exist?

Cavanaugh AM, Rauh MJ, Thompson CA, et al. - Since racial disparities in functional outcomes after TKA exist, researchers intended to determine if differences in rehabilitation utilization contribute to these disparities. Rehabilitation utilization was determined through linked Medicare claims data among 8,349 women recruited in the prospective Women’s Health Initiative cohort who had primary TKA between 2006-2013. Postacute discharge destination (home, skilled nursing facility, inpatient rehabilitation facility), facility length of stay, and number of home health and outpatient physical therapy (PT) sessions between racial groups were compared. Compared with non-Hispanic White women prior to surgery, non-Hispanic Black women had a worse physical function (median score: 65 vs 70) and a higher disability probability (13.2% vs 6.9%). According to findings, utilization of rehabilitation was generally similar between Black and White women who received TKA when accounting for need. No evidence of underutilization of post-TKA rehabilitation services was found, and therefore differences in post-TKA functional results do not seem to be a result of the inequitable receipt of rehabilitation care.
