Regional cluster of vanishing gastroschisis: A comparative study of antenatal and postnatal outcomes

Granger J, et al. - Via retrospectively assessing records for all cases of vanishing gastroschisis from May 2014 to May 2015, researchers examined the spectrum of pathology, antenatal ultrasound characteristics and postnatal outcomes. The tertiary centre provides antenatal and postnatal care for major congenital anomalies in a population of 1.6 million. Cases of normal variant gastroschisis were used for comparison. Using the Mann–Whitney U‐test, maximum antenatael bowel diameter measurements were compared. During the study period, six infants with vanishing gastroschisis were born, accounting for 50% of all live‐born gastroschisis. According to this comparative study, vanishing gastroschisis is a serious, often catastrophic variant of gastroschisis. Aetiological factors contributing to the recent high incidence of this rare complication in the newborn population remain unclear, leading to secondary prevention strategies for the recovery of the midgut. For fetuses with intra‐abdominal bowel dilatation > 10 mm, the authors propose closer antenatal surveillance to prompt consideration of earlier delivery to improve morbidity and mortality.
