Reducing caesarean delivery: An economic evaluation of routine induction of labour at 39 weeks in low-risk nulliparous women

Callander EJ, et al. - Researchers conducted the study for evaluating the cost-effectiveness of three different interventions known to decrease caesarean delivery rates compared with standard care and conducting a budget impact analysis. In order to compare the costs and outcomes produced by the different interventions, a Markov microsimulation model was constructed. All interventions, plus standard care, produced comparable health results. Caseload midwifery was the lowest cost option at $15,587, followed by routine induction of labour, and chart audit. On average, they all produced lower costs than standard care. If applied for all low-risk nulliparous women in Australia, caseload midwifery will deliver the greatest savings of $172.6 million over three years. Caseload midwifery offers the best value of the choices considered for minimizing caesarean delivery rates. Routine induction of labour at 39 weeks and chart audit would also lessen costs relative to standard care.
