Rates over time and regional variation of radical minimally invasive surgery for cervical cancer: A population based study

Willows K, et al. - Rates of radical minimally invasive surgery (MIS) for cervix cancer in Ontario, were determined in addition to assessing variation in these rates over time and by region. Furthermore, an investigation was done to assess how changes in the use of MIS has influenced the length of hospital stay and readmissions. Researchers performed a retrospective population-based cohort study including 805 women who underwent radical abdominal surgery and 538 women who underwent radical minimally invasive surgery. During the study period, they noted an increase in radical MIS, from 17.7% in 2002 to 61.5% in 2015; this appeared to have been driven by a few high volume centers. A significantly shorter mean length of hospital stay was evident following radical MIS vs radical abdominal surgery (1.1 vs 4.2 days). Over the study period, a reduction in hospital readmission within 30 days for MIS was observed but these remained stable following abdominal surgery. Observations suggest that the skills to carry out the procedure effectively are acquired with time. Surgical training and subsequent proficiency in the skills unique to this procedure might be significantly impacted with the abandonment of minimally invasive radical hysterectomy.
