Rapid start of antiretroviral therapy in youth diagnosed with HIV infection

Patel ND, Dallas RH, Knapp KM, et al. - Given the recommendation of initiating antiretroviral therapy (ART) as soon as possible in adults and adolescents with HIV, researchers sought to describe this approach for youth. On April 1, 2018, ART initiation at the first clinic visit was started among youth with HIV infection at St. Jude Children’s Research Hospital. They here documented the results of a quality improvement initiative that compared patients who were offered ART at their first visit to a historical cohort of patients who initiated ART at a subsequent visit. During the indicated time period, 124 ART-naive youth (median age 19 years, 91% male, 94% black) were reported to be first seen. The baseline cohort comprised a total of 54 patients and the rapid start cohort comprised 70 patients. Findings support the feasibility of starting ART-naïve youth with HIV infection on ART at their first clinic visit, along with its high acceptance and its correlation with faster viral load suppression, and high retention in care.
