Randomized trial of adjuvant radiotherapy following radical prostatectomy vs radical prostatectomy alone in prostate cancer patients with positive margins or extracapsular extension

Hackman G, Taari K, Tammela TL, et al. - Via a randomized, open-label, parallel-group trial of 250 patients between April 2004 and October 2012 in eight Finnish hospitals, with pT2 with positive margins or pT3a, pN0, M0 cancer without seminal vesicle invasion, the researchers intended to contrast the efficiency and tolerability of adjuvant radiotherapy after radical prostatectomy. For patients who were alive when the follow-up ended, the median follow-up time was 9.3 yr and 8.6 yr in the adjuvant group and in the observation group, respectively. In the adjuvant group and in the observation group, the 10-yr survival for biochemical recurrence was 82% and 61% and for overall survival 92% and 87%, respectively. The occurrence of two and four metastatic cancers respectively was noted. Thirty-seven patients of the 43 patients with biochemical recurrence in the observation group got salvage radiotherapy. Grade 3 adverse events were experienced by 56% vs 40% in the adjuvant group and in the observation group. Just one grade 4 adverse event occurred (adjuvant group). The number of people was a limitation of this study. Hence, in patients with positive margins or extracapsular extension, adjuvant radiotherapy post radical prostatectomy was commonly well-tolerated and extended biochemical recurrence-free survival in comparison with radical prostatectomy alone.
