Radiation therapy after breast-conserving surgery in women 70 years of age and older: How wisely do we choose?

Downs-Canner S, et al. - Given the high radiation therapy (RT) usage for women ≥ 70 years of age with T1, estrogen receptor-positive (ER+) tumors undergoing breast-conserving therapy (BCT) despite data from randomized trials supporting omission of RT, researchers reviewed their institutional experience to determine if RT use is influenced by risk factors for local recurrence or comorbidities. They analyzed overall, 323 patients with 327 cancers and realized that only younger age and larger tumor size were associated with RT use. They observed recurrence-free survival of 98% vs 93% with and without RT, respectively. In addition, they noted improved overall survival in those who received adjuvant radiation (92% vs 89%), although this effect did not reach statistical significance. In these women, adjuvant radiation was thus noted to be neither influenced by the factors associated with the risk of local recurrence nor by the estimated risk of death in 10 years.
