Quantitative neuroimaging measures of myelin in the healthy brain and in multiple sclerosis

O'Muircheartaigh J, et al. - Among 56 patients with relapsing–remitting multiple sclerosis (MS) and 38 healthy controls, researchers compared four established myelin-sensitive magnetic resonance imaging (MRI) techniques: Multi-echo T2 relaxation, mcDESPOT (multi-component driven equilibrium single pulse observation of T1 and T2), magnetization-transfer ratio (MTR), and quantitative T1 (qT1). Variations between MS normal-appearing white matter (WM) and healthy control WM were demonstrated by all measures in eight WM tracts, with qT1 displaying the highest number of different regions (8), followed by MWF-D (the fraction of signal attributed to myelin water from mcDESPOT) and MTR (6), and MWF-G (n = 4; MWF measurement obtained using a GRadient And Spin Echo). The best accuracy was shown by MWF-D when the methods were compared with respect to their statistical sensitivity to MS lesions in WM. In this study, the average and standard deviation volumes of the four techniques, calculated from the healthy controls, is offered for future power analysis.
