Quality of life, cognitive, physical and emotional function at diagnosis predicts head and neck cancer survival: Analysis of cases from the Head and Neck 5000 study

Rogers SN, Waylen AE, Thomas S, et al. - Researchers investigated how the health-related quality of life (HRQOL) at diagnosis of head and neck cancer (HNC) influences overall survival following treatment with curative intent after adjusting for other factors. From the Head and Neck 5000 study (HN5000), data from 5,511 participants were assessed. They used the EORTC QLQ-C30 for measuring HRQOL. They identified the availability of questionnaire and covariate data from 2,171 participants who were diagnosed as follows: oral cavity (655), oropharynx HPV+ (723) and HPV− (277), and larynx (516). Following adjustment for demographic and clinical confounders, they identified a higher survival among people who report higher (better) levels of HRQOL at diagnosis vs those with lower self-reported HRQOL (associated with worse survival for all tumor sites). A similar finding was observed for both the global HRQOL score and all functional domains except emotional and cognitive functioning. Further, they identified tumor site-specific correlations between HRQOL and survival: higher global HRQOL scores at baseline are linked with enhanced survival rates for those with oral cavity and laryngeal cancer. Given that most people diagnosed with cancer have survival as the main priority, pretreatment HRQOL is an additional factor that should be included in risk stratification and case-mix adjustments. They identified incorporating HRQOL into routine clinical care as valuable given it as a useful facet in patient-clinician decision making, prognostication, and recovery.
