Prospective evaluation of an R2* method for assessing liver iron concentration (LIC) against ferriscan: Derivation of the calibration curve and characterization of the nature and source of uncertainty in the relationship

Jhaveri KS, et al. - In this prospective study, researchers identified an R2*-based MRI technique that might potentially be used as an alternative to FerriScan [the method-of-choice for noninvasive liver iron concentration (LIC) quantification]. This was achieved by deriving a calibration curve characterizing the relationship between FerriScan-derived LIC and R2*. In this curve, the nature and source of uncertainty were investigated. It has been assumed that the source of uncertainty is heterogeneity of LIC across the liver. During the same examination, 125 patients undergoing iron overload chelation treatment prospectively underwent FerriScan and R2* MRI. A linear relationship between FerriScan-derived LIC and R2* logarithmically transformed has been confirmed. Confirmed as iron heterogeneity across the liver was the source of uncertainty. In this investigation, it was found that FerriScan and R2* techniques provide equivalent quantification of LIC. As compared to the uncertainty caused by variation in LIC over the liver, any difference in accuracy or exactness was at a level lower.
