Proportion of US adults recommended out-of-clinic blood pressure monitoring according to the 2017 Hypertension Clinical Practice Guidelines

Booth JN, et al. - Researchers used the 2011 to 2014 National Health and Nutrition Examination Survey (n=9623) to determine the percentage of US adults meeting criteria for out-of-clinic BP monitoring by the American College of Cardiology/American Heart Association guideline. Among US adults not taking antihypertensive medication, criteria for out-of-clinic BP monitoring to screen for white coat hypertension were met by 92.6% with systolic/diastolic BP ≥130/80 mm Hg and criteria to screen for masked hypertension were met by 32.8% with systolic/diastolic BP<130/80 mm Hg. At an older age, they noted less often meeting the criteria for out-of-clinic BP monitoring to screen for white coat hypertension. They observed a higher proportion of individuals meeting the criteria for out-of-clinic BP monitoring to screen for masked hypertension at an older age, among men vs women and non-Hispanic blacks and whites vs non-Hispanic Asians or Hispanics. Among US adults taking antihypertensive medication, criteria to screen for white coat effect were met by 12.5% with systolic/diastolic BP ≥130/80 mm Hg and criteria to screen for masked uncontrolled hypertension were met by 57.4% with systolic/diastolic BP<130/80 mm Hg. At an older age and among non-Hispanic blacks vs non-Hispanic whites, they noted more commonly meeting to criteria for out-of-clinic BP monitoring to screen for white coat effect. In older adults and men, criteria for out-of-clinic BP monitoring to screen for masked uncontrolled hypertension were more commonly met. In total, there are about 103.8 million US adults (45.8%) who met the 2017 Hypertension Clinical Practice Guidelines criteria for out-of-clinic BP monitoring.
