Prophylactic low dose ketamine infusion for prevention of shivering during spinal anesthesia: A randomized double blind clinical trial

Thangavelu R, et al. - Researchers investigated whether intraoperative and postoperative shivering under spinal anesthesia could be prevented by administering prophylactic low dose ketamine bolus followed by infusion. This analysis involved 60 patients belonging to American Society of Anaesthesiologists 1 and 2 receiving abdominal and lower limb surgery, who were randomly assigned to receive either 0.2 mg/kg iv of ketamine bolus followed by infusion 0.1 mg/kg/hr (Group K) or 5 ml of saline followed by 0.1 ml/kg/hr solution (Group S) as an infusion throughout surgery period. In the saline group and in the ketamine group, intraoperative shivering developed in 18 patients (58.06%) and in only 4 patients (13.79%), respectively. Also, significantly less postoperative shivering was reported in ketamine group vs saline. Overall, findings demonstrated the effectiveness of prophylactic low dose ketamine, delivered as a small bolus followed by an infusion, in preventing both intraoperative and postoperative shivering.
