Prophylactic cefazolin concentrations in morbidly obese patients undergoing sleeve gastrectomy: Do we achieve targets?

Grégoire M, et al. - In view of the fact that morbid obesity increases the risk of surgical site infections, researchers built a population pharmacokinetics model for a morbidly obese population to assess a 4000-mg dose of cefazolin recommended by the guidelines using Monte Carlo simulations and proposed new administration schemes. As per observation, the 4000-mg dosage was usually sufficient for prophylactic cover for 2 h. Continuous infusion could be required for minimum inhibitory concentration up to 4 mg/L. The 2000-mg and 3000-mg dosages were observed not achieving sufficient concentration.
