Propensity matched comparison of in-hospital outcomes of TAVR vs SAVR in patients with previous history of CABG: Insights from the Nationwide inpatient sample
Catheterization and Cardiovascular Interventions Dec 13, 2018
Nalluri N, et al. - Researchers compared in-hospital outcomes among patients ≥ 18 years with prior coronary artery bypass grafting (CABG) who underwent either transcatheter aortic valve replacement (TAVR) (35.05 and 35.06) or surgical aortic valve replacement (35.21 and 35.22). These participants were identified from 2012-2014 Nationwide inpatient sample (NIS) by using the International Classification of Diseases-Ninth edition, Clinical Modification (ICD-9-CM) codes. They also performed propensity score matching (1:1) and compared these outcomes between matched cohorts. They noted in-hospital mortality rates did not differ significantly. A lower risk of stroke (1.2% vs 3.3%, P = 0.009), AKI (12.9% vs 21.3%, P < 0.0001), myocardial infarction (0.9% vs 2.7%, P = 0.01) and major bleeding (9.1% vs 25.1%, P < 0.0001) was observed with TAVR vs SAVR. Also, they noted higher risk of pacemaker implants and lower risk of vascular complications with TAVR.
