Prognostic value of nodal response after preoperative treatment of gastric adenocarcinoma

Sada YH, et al. - Given that poor survival in patients with gastric cancer has been reported in relation to pathologically positive lymph nodes (ypN+) following preoperative chemotherapy, researchers examined the link between preoperative therapy response and postoperative therapy benefit in this retrospective cohort study including patients who have received preoperative therapy followed by surgery (2006–2014) for clinically node-positive (cN+) gastric cancer in the National Cancer Database. Using multivariable Cox regression, assessments were carried out to determine the link between overall risk of death and preoperative treatment, disease response, and adjuvant therapy use. Pathologic nodal response with preoperative treatment was shown by more than one-third of node-positive gastric cancers. A higher response was seen with radiation therapy vs chemotherapy. Worse survival was observed in patients with ypN+ disease, irrespective of whether they have postoperative therapy.
