Prognostic value of impaired hepato-renal function assessed by the MELD-XI score in patients undergoing percutaneous mitral valve repair

Spieker M, et al. - In 192 consecutive patients undergoing percutaneous mitral valve repair (PMVR) with the MitraClip system, the utility of the Model for End-stage Liver Disease (MELD)-XI score in the prognostication of these patients was assessed. On the day of the procedure, researchers determined the MELD-XI score which was used to define three groups of patients that were compared in terms of clinical features and outcomes after PMVR. A high-risk preoperative assessment was needed in patients with a MELD-XI score > 16 as these subjects have high mortality. Furthermore, given the considerable one-year mortality following PMVR, the best treatment option should be carefully discussed with these patients within the heart team. The best predictor of one-year all-cause mortality was a cutoff of 16 in the MELD-XI score, according to Youden-Index.
