Prognostic significance of crazy paving ground grass opacities in non-HIV Pneumocystis jirovecii pneumonia: An observational cohort study

Kumagai S, et al. - In this retrospective review of 61 patients diagnosed with non-human immunodeficiency virus (HIV) Pneumocystis jirovecii pneumonia (PjP), researchers analyzed high-resolution computed tomography (HRCT) findings that included ground grass opacities (GGO) types and assessed clinical outcomes of these patients in relation to crazy paving GGO. Those with or without crazy paving were included in the GGO types. They used Cox regression model to evaluate the links between HRCT findings and in-hospital mortality. At diagnosis of non-HIV PjP, HRCT-detected GGO was found in all patients. Crazy paving GGO (29 patients, 47.5%), consolidations (23 patients, 37.7%), bronchiectasis (14 patients, 23.0%), and centrilobular small nodules (30 patients, 49.2%) were all detected on HRCT. A poor prognosis could be seen among patients with crazy paving GGO on HRCT imaging and low serum albumin levels at the diagnosis of non-HIV PjP.
