Prognostic significance of blood and lymphatic vessel invasion in pathological stage IA lung adenocarcinoma in the 8th edition of the TNM classification

Samejima J, Yokose T, Ito H, et al. - In this retrospective analysis, researchers assessed the prognostic value of blood and lymphatic vessel invasion in p-stage IA lung adenocarcinoma in the 8th edition of the Tumor, Node, Metastasis (TNM) classification. They analyzed overall 306 patients with p-Stage 0-IA lung adenocarcinoma, re-grouped as per the 8th edition of the TNM classification. A median follow-up of 98.0 (range: 10-216) months was performed. They found significant differences in 5-year recurrence-free survival among patients with and without tumor vessel invasion (TVI) in p-stage IA1 and IA2 but not in p-stage IA3. Also, significant differences were evident for 5-year lung cancer-specific survival among those with and without TVI in p-stage IA1 and IA2 but not in p-Stage IA3. In this study, TVI was identified as a prognostic factor in patients with p-stage IA1-2 lung adenocarcinoma, and therefore, P-stage IA1 lung adenocarcinoma without TVI may be categorized as minimally invasive.
