Prognostic implications of normal or minimal urinary findings on long-term renal impairment in adults with Henoch-Schonlein purpura

Baumrin E, Azzawi S, St. John J, et al. - In Henoch-Schonlein purpura (HSP) patients (n = 47) who present with normal or minimal urinary abnormalities, researchers conducted this retrospective cohort study to assess the risk of long-term renal impairment. Study participants included adult HSP patients presenting with normal urinalysis, microscopic hematuria or low-grade proteinuria. Patients with median follow up 73.9 months (IQR 35, 98) were included. The authors discovered that 39 patients had an abnormal urinalysis, 15 of whom progressed to renal impairment in the long term. The data presented in this work showed a poor prognostic sign for long-term renal impairment development is microscopic hematuria and low-grade proteinuria at HSP diagnosis. For prolonged surveillance, this population should be targeted.
