Prognostic impact of abdominal lymph node involvement in diffuse large B-cell lymphoma

Abdulla M, Guglielmo P, Hollander P, et al. - In a retrospective single-centre study, researchers determined the prognostic importance of nodal abdominal involvement in relation to tumour cell markers and clinical features of 249 diffuse large B-cell lymphomas (DLBCL) individuals. Contrast-enhanced CT of the abdomen and thorax showed pathologically enlarged abdominal lymph nodes in 156 individuals, on the other hand, in 93 patients there were no pathologically enlarged lymph nodes in the abdomen. In 81 cases the DLBCL diagnosis was confirmed by histopathological biopsy obtained from abdominal lymph node. Compared with patients without nodal abdominal involvement, but less frequently extranodal involvement, those with abdominal nodal disease had inferior lymphoma specific survival and presented with higher age-adjusted international prognostic index, lactate dehydrogenase and more often advanced stage, bulky disease, B-symptoms, and double expression of MYC and BCL2. The worst finding was found in those where histopathological biopsy confirmed the abdominal nodal involvement. Patients with abdominal nodal disease with DLBCL had lower outcome and more aggressive behaviour, reflected in both clinical and biological characteristics.
