Prognostic factors for heart recovery in adult patients with acute fulminant myocarditis and cardiogenic shock supported with extracorporeal membrane oxygenation

Chou HW, Wang CH, Lin LY, et al. - Given that extracorporeal membrane oxygenation (ECMO) represents an effective support method for acute fulminant myocarditis (AFM) with cardiogenic shock, but controversy still exists about deciding if to bridge to a left ventricular assist device (LVAD) or to maintain ECMO support until heart recovery, so, researchers addressed this topic in this retrospective observational analysis at a single center with 88 adults with AFM who received ECMO support between 2006 and 2018. Heart recovery without heart transplantation or long-term LVAD support was considered as the primary endpoint. In this study, poor heart recovery was observed in relation to high CK-MB levels and ventricular tachycardia/ventricular fibrillation/asystole in patients with AFM. A potentially protective effect of early intravenous immunoglobin use was shown.
