Prognosis and neuropathologic association of clinical subtypes of Parkinson disease

De Pablo-Fernández E, et al. - Researchers investigated disease progression and neuropathology in relation to clinical subtyping of Parkinson disease at diagnosis. In this retrospective cohort study, clinical subtyping of 111 autopsy-confirmed patients with Parkinson disease based on motor symptoms, rapid eye movement sleep behavior disorder, and autonomic and cognitive dysfunction at diagnosis indicated that disease course and survival rates were different across subtypes. Parkinson subtypes displayed different rates of Lewy pathology and Alzheimer disease-related neuropathology progression. They observed earlier development of milestones and reduced survival in the diffuse malignant subtype. As per Cox proportional hazard regression, an increased adjusted risk of any disease milestone and death was noted in the diffuse malignant group.
