Prior pelvic osteotomy affects the outcome of subsequent total hip arthroplasty

Yacovelli S, Abdelaal M, Fillingham Y, et al. - In this study, the outcome of the conversion of total hip arthroplasty (THA) was ascertained in patients with prior pelvic osteotomy. Researchers matched 49 patients with a history of the prior pelvic osteotomy (PO) who had undergone conversion THA at a single institution in a 1:3 ratio for date of surgery, age, gender, and BMI with 147 patients with DDH who had undergone primary THA without prior PO. They conducted a retrospective chart review to compare outcomes at a minimum of 2 years follow up. It was revealed that after prior PO, THA is technically demanding, leading to longer operative times, greater blood loss, and variation in implant placement. The data indicated that THA after a prior PO is more likely to require reoperation, while functional outcomes are similar.
