Primary medial patellofemoral ligament repair vs reconstruction: Rates and risk factors for instability recurrence in a young, active patient population

Puzzitiello RN, Waterman B, Agarwalla A, et al. - In young patients with primary medial patellofemoral ligament (MPFL) repair or reconstruction, researchers comparatively assessed the clinical results and rates of recurrent instability, as well as evaluated for radiologic risk factors for worse consequences. The study sample consisted of all patients with lateral patellar instability who had either MPFL repair and/or imbrication or MPFL reconstruction without any additional osseous procedures between 2008 and 2015 at a single center. Fifty-one knees with isolated MPFL surgery were identified at a mean of 59.7 months' follow-up. According to this retrospective review, MPFL reconstruction can result in improved midterm clinical outcomes and a reduced recurrence rate vs MPFL repair. In patients who undergo isolated MPFL repair, increased patellar height as measured by the Caton-Deschamps Index can be a risk factor for recurrent patellar instability.
