Prevalence, characteristics and endoscopic management of gastric premalignant lesions in France

Chapelle N, Péron M, Mosnier JF, et al. - In the present study, researchers analyzed the modalities of endoscopic management of gastric precancerous lesions (GPL) patients in France. All patients diagnosed with GPL in the center between 2000 and 2015 were grouped and analyzed in the following order according to the most severe GPL found: atrophic gastritis only (AG), intestinal metaplasia (IM), low grade dysplasia (LGD), high grade dysplasia (HGD). Of the 16,764 patients with upper endoscopy with gastric biopsies, 507 patients were diagnosed with GPL (detection rate 3.2%). Overall, in 41% of patients, Helicobacter pylori infection was found. IM was by far the most common lesion detected (79%), followed by LGD (17%), HGD (2%) and AG (2%). GPL can be expected for any cause in France in about 3% of patients undergoing upper endoscopy with gastric biopsies. There is a weak link between endoscopic assessment and histology. In the future, the dissemination of published guidelines should improve patient management with GPL. There was no connection between the endoscopist’s appreciation of the mucosa and histological outcomes.
