Preoperative ultrasound planning in the surgical management of patients with hidradenitis suppurativa

Cuenca‐Barrales C, et al. - Given that ultrasonography can improve disease severity assessment in hidradenitis suppurativa (HS) patients, researchers evaluated changes in the projected skin excised area after the use of preoperative ultrasonography and analyzed the clinical factors potentially correlated with inaccurate clinical surgical delimitation. In addition, they contrasted recurrence rates after preoperative ultrasound vs clinical surgical margin delimitation at week 24. Patients for surgical treatment by wide excision and healing by secondary intention with a clinical projected skin excised area equal to or larger than 15 cm2 have been involved. Preoperative ultrasonography improves the delimitation of surgical margins and can lower the risk of recurrence in HS patients at 24 weeks. Obesity, male sex, Hurley II stage and elevated IHS4 scores are potential risk factors for inaccurate clinical surgical delimitation, so particular consideration should be paid to these patients when ultrasonography is not available and surgical delimitation is focused solely on clinical examination.
