Preoperative predictors of choledocholithiasis in patients presenting with acute calculous cholecystitis

Chisholm PR, et al. - Researchers carried out a retrospective review to define the range of presenting lab values and imaging findings in acute calculous cholecystitis (AC), develop a model for predicting the presence of concurrent choledocholithiasis (CDL) and develop a management algorithm that can be easily applied on presentation. Study participants were patients presenting with AC to a large tertiary hospital over a 3.5-year period. The prevalence of choledocholithiasis in patients with AC is high. When using a validated model, cutoffs for alanine transaminase (ALT), AlkPhos and common bile duct (CBD) diameter can effectively triage patients with a low and high probability of CDL to surgery or ERCP.
